Nah, that's from the chip. I go entire play throughs without ever eating (think, 30 in game days minimum) and never seen this. So I looked it up and it's a chip malfunction.

For those curious, I believe this only randomly begins to happen once you approach a mirror past Act 2 and into Act 3, due to the Relic’s influence.
In fact, if you use the shower, occasionally and randomly V will end up coughing up blood mid shower and wash it off, though you still keep the Stamina Boost from Showering.

The second reason why V raided the Arasaka tower was because they were known to be the only place on the West Coast that had snicker bars in their vending machines, hence why V takes a bite out of one in the Star and Sun endings.
