United Airlines Flight 811 was a regularly scheduled airline flight from Los Angeles to Sydney, with intermediate stops at Honolulu and Auckland. On February 24, 1989, the Boeing 747-122 serving the flight experienced a cargo door failure in flight shortly after leaving Honolulu. The resulting explosive decompression blew out several rows of seats, resulting in the deaths of nine passengers. The aircraft returned to Honolulu, where it landed without further loss of life.

the falling cargo door thing lasted for 20 years, and was known for damaging the reputation of a plane that later became known as the execution chamber.
Things are really only going to change when people refuse to travel by air until proper safety replacements and maintenance are done to the planes. But that's never going to happen.

Ive noticed that the Captains who have all survived major catastrophies have atleast one thing in common. They all broke protocol before asking for permission to do so. Examples being dumping fuel, dropping altitude, landing without permission and starting the APU immeadiatly instead of following the checklist.
I don't know what is more amazing. The negligence that let a problem be ignored until it was too late or the pilots and plane that actually managed to hold on to come out relatively well all things considered.
