Caused a nuclear chain reaction? All operating reactors have nuclear chain reactions. Maybe you meant an uncontrolled nuclear chain reaction that caused all the reactor water to turn to steam, causing rapid destruction of the reactor in a steam explosion.

history is clearly the escape of the COVID-19 virus from the Wuhan Institute of Virology
in Wuhan China which has cost an estimated 25 TRILLION dollars and 6,820,000 lives.
Another engineering mistake: the capsizing of the oil platform Alexander Kielland killed 123 Norwegians.

Time and again in history, Ukrainians have been incredibly suffer over and over again just because they had Russia for a neighbor/dominator. Bulk of millions of WWII deaths on eastern front were Ukrainians. Most of the millions of deaths of Stalin's famine were Ukrainians. The majority of Chernobyl disaster fell on Ukrainians. And now the war also falls on Ukrainians. I feel so sorry for them.
