You forgot Buddy Ebsen was the original Tin Man in Wizard of Oz. He had an allergic reaction to the paint they used and almost died. They replaced him w/ the actor we know now

You forgot to mention when Stallone wanted Ernie Shavers to play clubber lang in rocky 3 and in an audition asked Shavers one of the hardest punching heavyweights of all time hit Stallone, Ernie after repeatedly saying no finally did it it, not as hard as he could in the liver. Stallone immediately stopped was rushed into the mens room had someone tell Shavers they could not use him and nearly died from the blow.

I had to leave during "The Passion of The Christ" because I was so upset and crying. My husband helped me out of the theater. It was just too heartbreaking.

Honorable mentions, you add in the list Michael J Fox since he was slowly hanged, but add Brendan Fraser in honorable mentions when he almost snapped his neck in his hanging scene?
Fraser came so much closer to die then Fox if you compare those 2 scenes.
